Job Description Job Description GENERAL PURPOSE: Responsible for
the development and implementation of person-centered treatment
plans that seek to resolve the current presenting difficulty and
support the individual in stepping down to less intensive community
settings as quickly as possible. Additional support in the areas of
medication management, resource connection (e.g., housing
resources, AOD, PCP, OP, etc.), counseling and advocacy will be
provided as needed. DISTINGUISHING CHARACTERISTICS This is an
at-will direct service position within a program. The position
employee is responsible for assisting and advocating for our
members in all areas of treatment and helps them in addressing
barriers that prevent them from achieving chosen meaningful roles
as well as supporting opportunities for clients to make informed
decisions regarding their mental health treatment in order to
enhance quality of life roles. ESSENTIAL DUTIES AND
RESPONSIBILITIES – (ILLUSTRATIVE ONLY) The duties listed below are
intended only as illustrations of the various types of work that

related or a logical assignment to this class. In the absence of
the management team, provides leadership and direction or
clients/staff as assigned. Empowers clients in meeting their goals.
Identifies, researches, advocates for and assists clients in
accessing and receiving mental health treatment, medical care,
housing, food and clothing and other resources as indicated.
Coordinates intakes. Responds to referral requests and works
closely with hospitals and community providers to ensure smooth
transitions of clients into the program. Ensures that all admission
eligibility criteria are met and that appropriate documentation is
received to facilitate new admissions. Maintains all client records
and completes required documentation and data entry according to
program standards including progress notes, activity reports, logs,
etc. Develops and implements activities, programming, and groups
that promote wellness and reduce propensity for recidivism.
Supports clients’ medication therapy through encouraging adherence
to medication schedules as ordered by their doctor; supports
clients’ efforts at improvement of daily functioning by modeling
communication and behaviors that support the development of
naturally occurring relationships. Supports clients in completing
applications, setting appointments, and following through with
appointments to other community support agencies. Interacts
productively within the team in order to provide holistic case
management services to an identified group of clients. Attends to
the safety, health, and well-being of clients. Completes paperwork
as assigned, accurately and in a timely manner including but not
limited to: Daily Progress Notes, Service Plan, Discharge, LOCUS
assessments, Intake assessments, etc. Provides on-the-spot
counseling and family interventions as needed; Provides prompt
intervention in the event of a crisis both to stop the crisis and
to notify when indicated, persons and agencies necessary for the
resolution of the crisis. Facilitates family support meetings to
support the transition of clients into their naturally occurring
support systems. MINIMUM QUALIFICATIONS AND CERTIFICATIONS &
LICENSES Education, Training and Experience: A typical way of
obtaining the knowledge, skills and abilities outlined above is
through graduation from an accredited college or university with a
master’s degree in Social Work, Psychology, or related field and
two (2) years varied experience as a provider of mental health
services; valid registration with the Board of Behavioral Sciences
(BBS) LICENSES; CERTIFICATES; SPECIAL REQUIREMENTS Valid
registration with the Board of Behavioral Sciences (BBS).
California driver’s license & current vehicle
insurance/registration Reliable means of transportation capable of
passing vehicle safety inspection if more than five years old
excluding all modes of two-wheeled transport inclusive of bicycles,
mopeds and motorcycles. Schedule: Monday - Friday, 8:30 AM - 5:00
PM Compensation: $34.00 - $37.50 an hour. Interested? Join us at
