Physical Therapist Location: Walnut Creek, CA Agency: Cynet Health Pay: $3,051 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 4/27/2026 About the Position Job Title: Physical Therapist - Home Health Profession: Therapy/Rehabilitation Specialty: Physical Therapy Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8.5 Experience: Minimum 1 year of Home Health experience required License: California License in hand Certifications: BLS required Must-Have: - Oasis experience required - HCHB experience required Description: Position: Physical Therapist for Home Health. Schedule: Monday to Friday, 8:30 AM to 5:00 PM with a 30-minute lunch. No on-call requirements. Territory: Covers a 50-mile radius from the hub office. Average number of patients per day: 5-7 patients. Productivity expectation: 30 points per week. Types of cases: Focus on geriatric home health. First-time applicants will not be accepted. Weekend requirement: None. Mileage will be reimbursed at the IRS standard rate between patient homes. Local candidates are accepted at the travel rate.
