Day Shift Job Highlights - Competitive Pay: $75/hr – $85/hr - 13-Week Contract - Day Shift (8:00 AM – 4:30 PM) - 5 Days/Week Every Other Weekend - Work in a fast-paced acute care environment Schedule Overview Traveler 1: - Week 1: Mon, Tue, Wed, Fri, Sat - Week 2: Sun, Mon, Wed, Thu, Fri Traveler 2: - Week 1: Mon, Tue, Thu, Fri, Sat - Week 2: Sun, Tue, Wed, Thu, Fri Role Overview Join a dynamic care coordination team where you will play a key role in utilization review, discharge planning, and patient transitions. You’ll collaborate closely with physicians, multidisciplinary teams, and families to ensure safe, efficient, and timely patient care transitions. Key Responsibilities - Perform utilization reviews (pre-admission, admission & concurrent) - Lead and coordinate discharge planning - Collaborate with physicians & interdisciplinary teams - Facilitate care transitions & continuity of care - Educate patients, families, and staff on care plans - Ensure compliance with regulatory guidelines & healthcare policies ? Requirements - Active California RN License - BLS Certification - Minimum 2 years experience in: - Case Management - Utilization Review - Discharge Planning - Recent acute care / hospital experience required - Strong communication, critical thinking & coordination skills ?
